Member Agreement, Indemnity and Waiver of Liability:
- By purchasing below, I request the player listed below to be admitted as a member of UOVA for the 2011 season. I take responsibility for inspecting the playing area at each UOVA event in which they participate, and I hereby release, waive, discharge and covenant not to sue the Utah Outdoor Volleyball Association (UOVA), Tournament Directors or their employees, operators, promoters, officials, sponsors, advertisers, owners and lessees of premises used for UOVA events, other participants or and persons in the playing area, all for the purposes herein referred to as “Releasees”, from all liability to the undersigned, his/her personal representatives, assigns, heirs, and net of kind for any and all damage, and any claim or demands thereof on account of injury to the person or property or resulting in death of the undersigned, whether caused by the negligence of the Releasees or otherwise while the undersigned is in or upon the playing area or in any way competing, officiating, observing, working for, or for any other purpose participating in a UOVA event.
- By purchasing below, I hereby agree to indemnify and save and hold harmless the Releasees of any loss, liability, damage, or cost the player may incur due to the presence of the undersigned in or upon the playing area or in any way competing, officiating, observing, or working for, or for any purpose participating in the event and whether caused by the negligence of the Releasees or otherwise.
- By purchasing below, I hereby assume full responsibility for the risk of bodily injury, death or property damage of the player listed below due to the negligence of Releasees or otherwise while in or upon the playing area and/or while competing, officiating, observing, or working for, or for any purpose participating in the event.
- By purchasing below, I expressly acknowledge and agree that the activities at the event an in the playing areas are potentially dangerous and could involve the risk of serious injury and/or death and/or property damage.
- By purchasing below I further agree that the foregoing release, waiver and indemnity agreement is intended to be as broad and inclusive as is permitted by the law of the State of Utah, and that if any portion thereof is held invalid, it is agreed that the balance will continue in full legal force and effect.
- By my signature below I consent to and will permit emergency medical treatment of the player listed below if required. I also give permission for UOVA to use their name or photograph for promotion or advertising purposes.
- By purchasing below I certify that I am the legal gaurdian of the player listed below, and I have read and understand this release and waiver of liability and indemnity agreement, which embraces each and every event sanctioned by UOVA during the 2011 season, and that I voluntarily sign this application for 2011 membership.
- By purchasing below the player listed agrees to abide by the UOVA Code of Conduct at any and all UOVA events, and I accept that UOVA will take action again me or the player including possible suspension or forfeiture of my UOVA membership for any violations of the UOVA Code of Conduct.
NOTE! The checkout will ask for your shipping information. This is for billing purposes as well as to make sure you are included on our tournament information mailings. Your UOVA membership materials WILL NOT be mailed to you, they need to be picked up at your next tournament. Online price includes $1 fee.
